Subjective:

The patient is a 69 y.o. female that was seen for annual recall, 10 th year following surgery.

Objective:

The patient has no known drug allergies. There are no serious illnesses. Patient had under gone HA (six .8cc syringes Interpore 200 - 425-1000 microns) mandibular alveolar ridge augmentation and mandibular staple implant in 1988. A cholecystectomy was performed in 1997.

Assessment:

  1. status post Mandibular Alveolar Ridge Reconstruction with Hydroxyapatite and Staple Implant
